Transferring to Insulin Pens (ONLY FOR THOSE ON INSULIN PUMPS)
Please keep a record of your basal rates and total daily basal dose, insulin to carbohydrate ratio, and
insulin sensitivity (correction) factor.

Download your pump settings at home at https://diasend.com
(Ominipod, Tandem and Roche pumps) or https://carelink.minimed.eu (Medtronic pumps).

You must have long-acting (basal) insulin e.g. Abasaglar, Humulin I, Lantus, Levemir, Toujeo, Tresiba
available to you in the event that your pump does not work.

To calculate your long-acting insulin dose look for your total daily basal dose.

If you do not have long-acting insulin available, and your pump does not work, you will need to check your
blood glucose regularly and inject quick-acting insulin every 3-hours.
