Pre MRCP (Junior clinical fellow -JCF).

This post is a rotation through various specialties (cardiology, stroke medicine, care of elderly, diabetes and endocrinology, metabolic medicine etc) During this rotation you will be mainly managing patients on the wards, doing on calls. You will get to attend some out patient clinics. You will be expected to train for your MRCP examinations, attend training days, acute medical meetings. You will be expected to participate in teaching, presentations, audit and perhaps research. You will have an eportfolio to record your competencies to progress at each annual appraisal.

Consider this as time to prepare towards your membership examinations.

There are two possible options for  posts at this level.

  1. Medical posts These are posts based on Medical wards. This can be care of elderly, rehabilitation, Gastroenterology or other core medical specialties. Assuming you have already done the membership, you can have 2 sessions per week (when you are not on call) to attend medical specialty ward rounds or clinics. This is also an opportunity to demonstrate interest (et audit/ Research).
  2. Specialty posts. These can only be provided when they are available. These are posts based exclusively in a specialty (eg Gastro/ cardio/ Care of elderly). You will need to have completed your membership examination for these posts. You will attend specialty clinics/ procedures

Your on calls will still be in General Medicine. There will be study days, presentations and research. You will be supervising the JCF/ ST1/ ST2 /(Speciality Trainee)/CMT (core medical trainee).

 

 

The two posts below are only for those who have been able to demonstrate academic excellence, publications and year on year progression. These posts cannot be done on the MTI but are more for those on Tier 2 visa

Locum Consultant

Starting straight as a consultant without UK experience is fraught with difficulty. You might come over on a locum consultant contract, but it would be useful to spend 6 months getting used to the system. Please remember you will still need GMC registration and IELTS, but you do not need to be on the Specialist register. You cannot get a permanent consultant contract till you get on the specialist register. You can work as a specialty doctor in relevant specialties and then apply on the Article 14 (CESR).

Permanent Consultant post

Working as a consultant involves a considerable amount of administrative and developmental responsibility as well as teaching, training, and research duties. You will be expected to develop your department, keep up to date with knowledge and skills. Your ability to deal with clinical cases and deal with complexities of UK health and social care is expected as standard.
